Position: Full-time Outpatient Infusion Unit Case Manager

Summary:
The case manager is responsible for coordination of services through the continuum of care, balancing quality patient care with cost-effective utilization of resources.
Responsibilities:
• Coordinate client-centered care that is safe, effective, efficient, and equitable.
• Develop effective working relationships with the medical team throughout the case management process.
• Handle case assignments, review case progress and determine case closure.
• Assist patients in making informed decisions by acting as their advocate regarding their treatment options.
• Communicate with the patient’s health plans through the pre-certification process to secure reimbursement for the hospital services rendered.
Qualifications:
• Minimum Associates Degree in Nursing required.
• 2 years case management experience preferred.
• Excellent communication, organizational, and time management skills.
• Excellent knowledge of case management principles, healthcare management and reimbursement.
